POTTS Chart
Introduction
The Physiological Observation Track and Trigger (POTTS) System Chart fully integrates physiological observations with early warning scoring (EWS), and eliminates the need for nurses to calculate the physiological score.
The Problem
Early warning scoring systems (EWSS) or track and trigger systems help to detect patients early in the course of their deteriorating condition.
Assessment of these tools has generated poor audit reports. Research at the University Hospitals of Morecambe Bay NHS Trust demonstrated that errors in observation recording and EWS calculation were systematic and not just human error.
The Solution
University Hospitals of Morecambe Bay NHS Trust established a multi-disciplinary team to design and trial a new observation chart (POTTS Chart) to successfully incorporate an EWSS.
A trial of the POTTS Chart has demonstrated that the chart offers an improved compliance and accuracy of EWS (90%; a two-fold improvement on the two previous years’ audits).
The Process
University Hospitals of Morecambe Bay NHS Trust approached TrusTECH for assistance in promoting adoption of the chart among other NHS Trusts.
TrusTECH distributed information about the POTTS Chart via its Service Innovation Bulletin, website and Service Innovation Showcase (December 2006). TrusTECH also provided a bursary to enable the University Hospitals of Morecambe Bay NHS Trust to hold a workshop for other Trusts that were interested in adopting the chart. This workshop provided background information on the development of the chart, the benefits of the chart, and provided information about how the chart could be integrated into various clinical areas within a Trust.
To date approximately, seven Trusts across England and Ireland are now adapting the chart for use in their own services.
