North West NHS Innovation Awards - Application Form Instructions

This guide is to accompany the application form for the North West NHS Innovation Awards 2007. It is a step-by-step set of instructions on how to complete and return the application form for consideration by the judging panel. Those points with an asterisk relate specifically to the electronic completion and submission of the form.
You are advised to read the competition rules carefully, and complete all sections of the application form and thoroughly check through your application to ensure that information is correct before submitting it to TrusTECH.

1. Download Adobe Acrobat Reader*
If you cannot open, edit or save the application form, you may need to download Adobe Acrobat Reader version 8 at no cost.
2. Save the Form*
You are advised to save the form as a pdf on your computer or server before completing the form (please save the form using the date, the initial of your forename and your surname using the following configuration:
yyyyddmm ESmith.pdf.
3. Complete the Form Fields
To complete the ‘text’ fields within the electronic form click the cursor on the field and enter the relevant text. To select an option within the electronic form using the ‘tick box’ fields, click the cursor and a cross will appear in the centre of the box.
If completing the hard version of the form, please use clear handwriting and a black ball point pen.
Section A – Contact Details
- Please note that in the electronic version of the form text entered in the large boxes will appear in the centre of the box and move up as more text is added (a word-limit restriction automatically applies here).
Section B – Project Submission
- Please choose ONE category into which your project will be entered; further help on how to choose a suitable category can be found in the competition rules.
- Please provide a brief description of your project.
- Please note that in the electronic version of the application form text entered into boxes 3–7 will appear in the centre of the box and move up as more text is added (a word-limit restriction automatically applies here) .
Section C – Data Collation
- For Questions 1 and 3, please choose as many answers as are applicable.
- For Question 2, please choose one answer.
4. Save the Completed Form*
To save the completed electronic form select ‘file’ and ‘save’.
5. Print a Copy of the Form for Your Records
You can print a copy of the completed electronic form for your records, by clicking the ‘print’ button at the top right of the first page. TrusTECH also advises applicants submitting hard copies of the form to retain a copy for their records.
6. Submit the Form
The closing date for entries is Friday 28 September at 16.00. Applicants are encouraged to submit their entries early (before 14 September) to go into a prize draw to win one of several branded USB Drives.
If you are submitting the form electronically, checked through your form thoroughly, saved it and then close the file. Attach the completed form (as a pdf file) to an email message (with the subject heading: Competition Entry) and send to Emma Smith, TrusTECH Communications Officer.
Alternatively, you can submit your application by post. Please send six hard copies of the completed application form to Emma Smith, TrusTECH Communications Officer, TrusTECH, the Innovation Unit, Manchester Royal Infirmary, Oxford Road, Manchester, M13 9WL.
7. Acknowledgement of Receiving Application
You should receive an email to acknowledge receipt of your application within one working day. If you do not, please call 0161 276 6964. Note if you do not receive an email receipt and do not contact TrusTECH, your application will not be entered into the competition.
